STATE OF OHIO MANDATED INFORMATION

In accounting for every student in the state of Ohio, we are required to report not only the school district in which our students reside but also the actual name of the school building where they would be if they were not attending OLSH (i.e., Princeton School District--Evendale Elementary School).  This is a new mandated state report that must be filed before May 10.

Parents of students in grades 1 through 6:

Please give this information to your child to give to his homeroom teacher,

who will record it on a class list and send it to me.

Thank you for your help with this.
